Submit a Claim - Claim Instructions

To qualify for a payment from the Net Settlement Fund, you must submit a timely and valid Claim Form. The Claim Form must be submitted online no later than January 6, 2022, or, if mailed, be postmarked no later than January 6, 2022.

You may download the Claim Form here, then submit the completed Claim Form:
